  Student Accident Plan
Date: August 2009 To: Parents of the Archdiocese of Baltimore Re: Catholic Archdiocese of Baltimore - Student Accident Insurance Plan - 2009-2010 School Year The Catholic Archdiocese of Baltimore is pleased to again offer a student accident insurance plan. A brochure which provides complete details regarding the plan can be reviewed and/or downloaded from the Archdiocese’s web site at http://www.archbalt.org/management-services/risk-management/student-accident-plan.cfm. As in the past, the student accident plan offers parents the option to purchase either School-time or 24-hour Accident insurance benefits. Parents interested in purchasing the coverage are encouraged to review details of the plan provided in the brochure on the Archdiocese web site, complete the enrollment form and mail the fully completed enrollment form to the Program Manager at the following address:
When mailing the enrollment form please include a check for the premium due. Checks should be made payable to Life Insurance Company of North America. Enrollment forms mailed without the premium payment or with a partial premium payment cannot be accepted. Questions regarding the plan can be directed to the Program Manager using either the toll-free telephone number or email address provided above. In addition, the Catholic Archdiocese of Baltimore, through the Office of Risk Management, will again automatically provide to all students enrolled in participating schools a catastrophic student accident policy with maximum benefits of $2,000,000*.
